All changes to In-Training Examination orders must be submitted via the Order Change Request Form. Change requests submitted by email or phone will not be processed.

Please use this form only for submitting changes to your In-Training Order Form. Please note, changes will not be made to your roster. Please allow three (3) business days for changes to be made.

Note: Any missing or incomplete information will delay the processing of your change request. You will be notified via email once the change has been made.

Please keep the following deadlines in mind:

The deadline to submit order changes for this year’s ABSITE, VSITE, and PSITE has passed. This form will not be available until next year.
